What to expect when you call the NSO Birth Certificate Delivery (02) 737-1111

1. You will be asked what NSO certificate you are requesting
a. Birth certificate (can only be requested by immediate family members) self, mother, father, son, daughter, parents parent, grandchild, authorized guardian, spouse
b. Marriage certificate
c. Death certificate
d. Certificate of No Marriage (CENOMAR)
2. You will be asked for personal information regarding your request
a. Full Name (follow information registered on the Birth Cert) > (maiden name for female applicants)
b. Birth date
c. Birth Place - complete town & province
d. Parent’s Name - mother's maiden name
e. Other Information -
i. any correction / legal proceedings made on the certificate - follow corrected info
ii. Specify purpose of application. If needed for abroad, indicate possible country of destination. If purpose is "Others" - indicate the particular purpose
iii. Indicate relationship of the requesting party with the owner of the certificate.
3. You will be given a reference number
a. You will use this number when you pay
4. You will be given payment center options with payment procedures
a. Metrobank
Fill-out a green payment slip:
Company name:NSO Helpline Plus
Account / Reference Number: 10-digit reference number
Account name: Requesting party's name
Amount: amount to be paid
b. Bayad Center
Fill-out the payment form:
Biller: NSO Helpline Plus
Date: Payment date
Account Number: 10-digit reference number
Amount due: Amount to be paid
Account name: Requesting party's name
c. Bancnet
1. Go to payment options
2. Choose NSO Helpline Plus
3. Enter the reference number and amount
(Please keep the transaction receipt for future reference)
5. After payment, your NSO certificate will be delivered to you within 3-4 working days
Processing period
more or less 3-4 working days - For Birth, Marriage, Death Certificates
more or less 7 working days (certificate of no marriage)
more or less 8 working days (recent event 2005 up to present)
Delivery Procedure:
If requesting party is NOT the owner, mother, father, son, daughter, parents parent, grandchild, authorized guardian, spouse of the birth certificate, He/She should obtain an Authorization to Request (ATR) from the owner authorizing him/her to request for the document.
