a. For New Passport Application

i. 3 pcs passport sized pictures with royal blue background
ii. One of the following IDs
1. Driver’s License
2. SSS ID
3. PRC ID
4. Voter’s ID
5. College ID (valid only if applicant is a College Student)
iii. NSO Birth Certificate printed in security paper
1. If late registered, provide any 2 of the following
a. Baptismal Certificate
b. Voter’s Registration Record from COMELEC Intramuros
c. School records - Form 137 (High School) / TOR (College)
iv. Supporting Documents with full name & birth details (any 1 or 2)
1. Marriage Certificate from NSO printed in Security Paper (SECPA) - required for married women and CFO Certificate of attendance if married to a foreign national.
2. Baptismal Certificate
3. PRC/IBP ID
4. Land Title - please delete
5. Government Service Record
6. Old SSS-E1 form - please delete
7. School form 137 or TOR
8. Voter’s Registration Record from COMELEC Intramuros
9. Seaman’s Book
10. Income Tax Return (old)
11. Other documents that show full name and birth details

b. For Passport Renewal Application:

For Green Passport:
i. 3 pcs passport sized pictures with royal blue background
ii. Original Old Passport to be renewed

Additional for Brown Passport:
i. NSO Birth Certificate printed in Security Paper.
ii. One of the original IDs mentioned above

3. You will be asked for a pick-up date of your payment and requirements

4. You will be scheduled for a Personal Appearance date

5. You will receive your new passport 8-10 working days from the date of your personal appearance date
